Team Calendars for Confluence add-on is unable to start

Symptoms
The Team Calendars for Confluence appears to be disabled after upgrading it and you can't manually enable it.
Cause
A bundled package is missing and the following stack trace appears in the logs:

- Rebuild the index using the UI.
